Contractor Registration Application

ELECTRICAL CONTRACTOR REGISTRATION REQUIREMENTS

  1. Current copy of supervising electrician’s license. It must be from a city, town or village in the State of Illinois that has an electrical commission that administers an examination.
  2. Copy of the supervising electrician’s driver’s license.
  3. Electrical Contractor Registration Affidavit completed and signed by the supervising electrician.
  4. A certificate of general liability in the amount of $1,000,000.00. Certificate holder must read: Cook County Department of Building and Zoning, 69 W. Washington, Suite 2830, Chicago, IL 60602.
  5. For corporations, proof of good standing with the State Of Illinois is required.

All documents requiring signatures must signed by the supervising electrician.
