Contractor Registration Application

Registration of all Trades under the Plumbing Department.

1. Illinois Plumbing Contractor's Registration 055-State Contractor Registration Certificate as issued by the Illinois Department of Public Health.
2. Applicant’s 058-State Plumber License or J-Chicago Journeyman Plumber’s License.
3. Applicant's State Licenses for Septic, Well, Fire Sprinkler, Irrigation Contractors.
4. On the Company Letterhead, Name, Address and Phone Number of three recently completed jobs,
   and on the same page below the Name, Address and Phone Number of three business references i.e. Bank, Supply House or General Contractor.
5. A copy of the previous year's Annual Domestic Corporation Report showing the list of officers.
6. Certificate of Insurance for a minimum of $1,000,000.00 liability naming Cook County Department of Building and Zoning as Certificate Holder only.
